By checking all boxes, I acknowledge and agree to the following terms and policies as a condition of participating in private lessons at PopRox Dance LLC (“PopRox”):

Assumption of Risk & Waiver of Liability *

I voluntarily assume all risks associated with my activities in a PopRox location, or those of my guests.

I release and hold harmless PopRox, its owners, instructors, employees, and affiliates from any and all liability, claims, or damages arising from my participation, whether caused by negligence or otherwise.

Pricing Agreement *

I agree to the private lesson rate of $100/hour base price, $20/hour for each additional adult, and $10/hour for each additional child

In the case I am redeeming the 10% bulk discount deal, I agree to pay in full for all lessons booked (minimum of 6).

In the case I request that my instructor learn or choreograph a specific or custom piece, I agree to pay an additional $40 choreography fee for each piece learned/created by the instructor.

If a custom music edit is required, I agree to pay the $25 music editing fee.

Full payment is due within 24 hours after the invoice is sent. A credit card on file is required for booking.

Policies *

Cancellation: Private lessons may be rescheduled no less than 24 hours before the lesson. Cancellations must be made no less than 72 hours prior, otherwise the full amount will be charged.

No-Show: Failure to attend a scheduled lesson will result in being charged in full. After two no-shows, PopRox reserves the right to discontinue scheduling future lessons.

Instructor Substitutions & Emergencies: PopRox reserves the right to reschedule or cancel lessons due to unforeseen circumstances (e.g., instructor illness, inclement weather, power outages). If rescheduling is not possible, a refund will be issued.

Communication & Consent

I agree to receive necessary communications about my lessons via email, phone, or text. I may opt out of marketing messages at any time.

Minor Participant Addendum: If the participant is under 18 years of age, this section must be completed by a parent or legal guardian.

I hereby authorize representative of PopRox Dance LLC, including employees, directors, or identified volunteers, to serveas agents for my child to consent to any emergency medical care, including x-ray exam, anesthetic, medical or surgical diagnosis or treatment and hospital care which is deemed advisable by and is rendered under the general or specific supervision of any licensed physician or surgeon on the medical staff of any hospital licensed by the State of Washington whether such diagnosis or treatment is rendered at the office of said physician or at said hospital or some other site. I also understand and agree that PopRox Dance shall not be held responsible for any injuries, damage, or personal loss which might occur while attending or participating in any PopRox function. The authorization shall remain valid for the duration of your child’s registration with PopRox Dance.
